For referral partners

Refer a patient with confidence.

For clinicians, hospitals, discharge planners, Employee Assistance Programs (EAPs), and therapists: when a patient needs detox or ongoing addiction treatment, Cedar Grove is a live, 24/7 front door in Louisville — with fast benefit verification and a coordinated handoff.

How referral works

Three steps, one phone call.

A direct line to a coordinator, and a clear answer on coverage and fit.

Call our admissions line

One call reaches a live admissions coordinator, any hour — 24/7, every day of the year. Give us the situation and we'll take it from there.

We verify benefits and fit

We confirm coverage and clinical appropriateness across our levels of care, and tell you whether Cedar Grove is the right setting for your patient.

Your patient is admitted

We coordinate a warm handoff into care, and — with your patient's written consent — keep you updated through the transition. Admission can happen as soon as the same day when it's clinically appropriate.

Coverage we work with

Kentucky Medicaid & major commercial plans

We're in network with Kentucky Medicaid and major commercial plans, and we verify each patient's specific benefits up front — confirming what's covered before admission.

What we provide

A continuum of care, under one roof.

Cedar Grove delivers medical detox and residential treatment through outpatient, IOP, MAT, and aftercare — so you know exactly what you're referring into. We do not run a partial hospitalization program (PHP).

Medical detox

Medically supervised withdrawal management with 24/7 nursing, as the first step when it's needed.

Residential treatment

24-hour residential care in a structured, supportive environment.

Outpatient & IOP

Outpatient treatment and an intensive outpatient program with morning or evening options for the step-down after acute care.

MAT within the program

Medication-assisted treatment with Suboxone, Subutex, or Vivitrol when clinically appropriate.

Co-occurring & aftercare

Treatment for co-occurring mental-health conditions, plus aftercare support for the step after treatment ends.

A front door that actually answers.

Your patient's window doesn't wait for business hours, and neither do we. Call our admissions team any time — we'll verify benefits, confirm fit, and coordinate the handoff with you.
